Metastatic spinal cord compression as an oncology emergency: getting our act together.

چکیده
BACKGROUND Metastatic spinal cord compression is a dreaded complication of cancer affecting 5-10% of patients requiring urgent treatment. A clinical practice improvement project was carried out to review and fine tune the clinical pathway for the acute management of patients with metastatic spinal cord compression. OBJECTIVE To improve the quality of care for metastatic spinal cord compression over 6 months by ensuring that >90% of patients receive definitive treatment within 24 h of radiological diagnosis. METHOD Using clinical practice improvement project methodology, the clinical pathway of 17 patients treated with radiotherapy for metastatic spinal cord compression within the last 6 months were reviewed to identify gaps and delays in the system. Interventions to form a multidisciplinary acute spinal cord crisis team, fine tune clinical referral processes and formulate a standardized treatment protocol were then implemented. Post-intervention of 22 subsequent patients were monitored for time to start steroids and radiation therapy, length of stay and hospitalization bill. RESULTS With the interventions implemented, the mean response time to start steroidal therapy was reduced from 8.4 to 2.6 days and radiotherapy from 9.9 to 3.9 days. These translated into shorter mean length of stay from 23.8 to 14.7 days and smaller hospitalization bill size from 13,723 to 8,808 Singapore dollars. CONCLUSION A clinical practice improvement project, to improve the quality of care for patients with metastatic spinal cord compression, can shorten response time to start steroidal therapy and definitive radiotherapy resulting in shorter length of stay and smaller hospitalization bill.

Oncology emergency modules: spinal cord compression.
SCC occurs as a result of a malignant neoplasm compressing the spinal cord. Malignant tumors may be classified as primary (i.e., arising from the tissue of the spinal cord or spinal canal) or secondary (i.e., arising as metastatic solid tumors) (Schafer, 1997). SCC most commonly is caused by metastatic tumors. Progression of symptoms is more rapid with metastatic tumors when compared to primary...
متن کاملModern approaches to the management of metastatic epidural spinal cord compression.
Metastatic epidural spinal cord compression (MESCC) is an oncologic emergency requiring prompt treatment to maximize neurologic function, ambulatory function and local control. Traditionally, options for MESCC included external beam radiation therapy with or without surgery. Surgery has usually been reserved for the patient with optimal performance status, single level MESCC or mechanical insta...

defined the triad of pathological fracture, spinal cord compression and need for surgery to bone as ‘morbid skeletal events’ (MSEs). With this analysis (Table 1), we demonstrate that while ZA demonstrated a significant delay in MSE compared with placebo (P = 0.004, there was no significant benefit of denosumab over ZA for both first and first and subsequent MSE (P = 0.07 and 0.35, respectively)...
متن کاملSpinal Cord Compression Secondary to Metastatic Sinonasal Undifferentiated Carcinoma
Sinonasal undifferentiated carcinoma (SNUC) is a rare malignancy of the upper airways and anterior skull base that carries a poor prognosis. The tumor is known to be invasive into the surrounding structures of the skull base and brain. To date, there is only one existing case report documenting drop metastasis to the intradural extramedullary spinal cord. To the best of our knowledge, we presen...
